He/she works with the primary doctor and agency staff to facilitate patient care activities; provide advice, support and assistance to the team members as needed and is responsible for ongoing patient/family evaluation and assessment.
The DON RN competently performs all basic nursing skills and conducts initial physical assessment of patient in the home.
Develops and maintains a written Care Plan for each patient and updates BI- monthly or more often if needed.
Collaborates with nursing staff to ensure the best care for the patient/family as established by physician.
The Director of Nursing will be responsible for the coordination of nursing services and as a liaison with appropriate management personnel and affiliate departments and committees, the development of policies and guidelines for home health clinical personnel.
He/she will direct and supervise professional and auxiliary personnel rendering patient care services and communicating the concept of quality client care to the general community.
The Director of Nursing will have primary responsibility for leadership and supervision to ensure that key bench mark indicators are met.
